WHAT IS A PERMANENT PARTIAL IMPAIRMENT RATING?
A Permanent Partial Impairment (PPI) rating is a standardized medical assessment that quantifies the degree of permanent functional loss resulting from an injury or illness. The rating is expressed as a percentage of whole-person impairment and is calculated using the American Medical Association (AMA) Guides to the Evaluation of Permanent Impairment, the most widely accepted framework for impairment assessment in the United States. PPI ratings are a critical component of many workers’ compensation proceedings and may be used to:
-
Determine permanent disability compensation
-
Support settlement discussions
-
Assess residual impairment after recovery
-
Assist administrative decision-making
-
Support claim closure and final disposition
When Is a PPI Rating Required?
A PPI rating is typically requested when an injured worker has reached Maximum Medical Improvement (MMI) — the point at which further significant recovery is not expected despite continued treatment.
At MMI, the evaluating physician determines whether permanent impairment remains and, if so, quantifies that impairment using accepted medical standards.
In Ohio, PPI evaluations are commonly used within the workers’ compensation system to help determine the appropriate level of permanent partial disability compensation.

COMMON REASONS FOR A PPI EVALUATION:
Claimant has reached Maximum Medical Improvement (MMI)
Permanent functional limitations remain
Impairment percentage must be determined
Administrative review is required
Settlement evaluation is being considered
Compensation determination is needed
