What Is a Medical File Review?

A medical file review is a service in which a physician reviews submitted medical records, imaging reports, diagnostic findings, and prior evaluations to render a written medical opinion — without conducting an in-person physical examination of the claimant. The reviewing physician analyzes the documentation and provides conclusions on specific clinical questions posed by the referring party.

When Is a File Review Appropriate?

When the claimant is unavailable or unwilling to attend an in-person examination, when the primary questions involve causation or treatment appropriateness rather than current functional status, when a cost-effective and faster alternative to a full IME is needed, for utilization review of proposed or ongoing treatment, or for second-opinion purposes on diagnosis and treatment plans.

Causation analysis — whether the condition is related to the claimed injury or exposure, treatment appropriateness — whether care received or proposed is medically necessary and guideline-consistent, diagnostic validity — whether the diagnosis is supported by the objective findings in the record, impairment and functional limitations based on documented findings, and consistency analysis — whether the claimant's reported symptoms are consistent with the objective medical evidence.
